The Pitt Worx and Talent Center Access Information Forms can be used to establish or modify access to Pitt Worx and Talent Center applications. These forms should only be completed and submitted for those individuals that need to be provided additional levels of access beyond the regular access provided to employees and supervisors or for those that need to have access modified/removed.

Pitt Worx Access Form

Instructions for Completing the Form

1. Download the Pitt Worx Access Information Form
   • Note: Please download the form prior to filling it out; do not complete the form in your browser.
2. Complete all relevant fields, including obtaining all necessary signatures from the supervisor and authorized RC approver.
   • Review a list of authorized RC approvers.
3. Submit the form to hrforms@pitt.edu.

Talent Center Access Form

Instructions for Completing the Form

1. Download the Talent Center Access Form (via Pitt Passport)
   • Note: Please download the form prior to filling it out; do not complete the form in your browser.
2. Complete all relevant fields, including obtaining all necessary signatures from the supervisor and authorized RC approver.
   • If any fields are left blank, the form will be returned to the department.
   • Review a list of authorized RC approvers.
   • Review the Talent Center User Role Access Guide (via Pitt Passport) for assistance with selecting the appropriate role.
3. Submit the form to hrforms@pitt.edu.
